Treatment — Hantavirus
No specific antiviral treatment
There is no FDA-approved specific antiviral treatment for hantavirus infection. Ribavirin has been studied but has not demonstrated consistent efficacy for HPS in controlled trials. Early administration of ribavirin may have some benefit for HFRS in some studies, but evidence remains limited.
Supportive care
Treatment is entirely supportive and must be initiated early:
For HPS
- ICU admission — patients deteriorate rapidly; continuous monitoring is essential
- Mechanical ventilation — for respiratory failure from pulmonary edema
- ECMO (extracorporeal membrane oxygenation) — for severe, refractory HPS; has improved survival in case series from 50% to nearly 70% when initiated early
- Haemodynamic support — careful fluid management (avoid fluid overload), vasopressors for shock
- Pulmonary artery catheterisation — to guide fluid management and cardiac output monitoring
For HFRS
- Fluid management — critical during hypotensive and oliguric phases
- Dialysis — for severe kidney injury with uraemia or fluid overload
- Blood products — for significant haemorrhage
- Electrolyte monitoring — particularly during the diuretic phase
No vaccine licensed in the Americas
- No hantavirus vaccine is licensed in the Americas, Europe, or most of the world
- China has licensed hantavirus vaccines (type I: Hantaan strain; type II: Seoul strain) since the 1990s; given to at-risk populations
- South Korea has a licensed Hantaan virus vaccine; recommended for farmers and military personnel
- Research into recombinant and DNA-based vaccines continues, but none are commercially available outside East Asia
Why early care matters
The single most important factor in hantavirus survival is early recognition and aggressive supportive care. Patients who reach an ICU before cardiovascular collapse have significantly better outcomes. ECMO availability has been associated with improved survival for severe HPS.
